Connected Care Center Privacy Policy and Supplemental Terms of Use
The Connected Care Center (“CCC”) platform allows you to access your Electronic Health Record (“EHR”) populated and sponsored by your medical services provider(s) (collectively, “Provider”). This Privacy Policy describes how CCC, PointClickCare Technologies Inc. including its subsidiaries and affiliates protect your personal information when you use this web site, the CCC and any related products and services (collectively, the “Services”).
Your use of the Services is subject to this Privacy Policy as well as the Terms and Conditions of Use for use of the CCC provided on registration, as well as the supplemental terms of use contained herein (together, the “Terms of Use”).
Information you Provide
When you create a CCC account, you are asked by your Provider to provide certain personal information, including your name a valid email address and your mobile number which will be shared with us. You will use your username and password for your CCC account. You also may be asked to confirm the information that you or a partnering Provider has provided to PointClickCare prior to registration, if any.
We use the information you provide to us to: confirm your eligibility for a CCC account; to protect against unauthorized access to your CCC account; to provide the Services to you; to communicate with you, and to provide assistance or technical support in connection with your use of the Services.
We use and store your information that you provide, to securely link your CCC account to your Provider and to provide the Services to you.
In general, PointClickCare collects all information that you supply directly to the Services. PointClickCare collects information from participating Providers whom you expressly authorize to use the Services with respect to you and your information. By authorizing a Provider, you also authorize PointClickCare to collect information regarding you from your Provider’s support staff and from other practitioners affiliated with your Provider or in your Provider’s practice. Further, PointClickCare may collect information from other third-party information providers that you expressly authorize to send information to your CCC account.
Please review this Privacy Policy and the Terms of Use (which incorporates this Privacy Policy) carefully. If you do not agree with our practices, do not access or use any part of the Services.
Information Collected by the CCC Website
There are two ways information can be submitted to us on the CCC website. The first is through direct submission or what we call ‘Manual Submission’ and the second is by way of ‘Automatic Submission’ triggered by any interaction with the CCC website (“Site”) through a computer, mobile device or tablet.
Manual Submission
Information is submitted to us directly when you use the Site to fill out a contact form, upload a file, answer survey questions, or provide any other information by filling out fields on the Site.
Automatic Submission
Whenever your computer, mobile device or tablet visits, logs in or otherwise interacts with the Site, the operating software of your device transmits a ‘request’ to us. That request includes non-personal information that is necessary to identify and route the information your device is requesting. This communication is necessary for all website and Internet services.
Additionally, we use certain analytic tools to track and analyze page usage behavior to improve performance in the use of the Site. We use this to track only what pages you visit and where you navigate, and do not use it to track any Personal Information (other than your email address for the purpose of optimizing user experience and to assist in the detection of any malicious email activity).
Please note that we do not alter our Site’s data collection and use practices when we see a “Do Not Track” signal from your browser. For greater clarification, while our data collection practices won’t be altered; they also won’t be applied in situations where you have prevented the tracking from occurring.
We may track and analyze non-identifying, aggregate usage, and volume statistical information from our visitors and customers and may provide such information to third parties whose services we utilize to administer our website, such as Google Analytics.
We do not sell your Personal Information.
Cookies
The Site uses a persistent cookie to provide you with a better user experience. We issue a session cookie to record information for the duration of a specific session visit or series of visits. The session cookie does not include the username or password of a customer. If you visit a part of our Site where we deploy cookies from LinkedIn, Facebook, Twitter, etc., their privacy policies will also apply.
Cookies are used in the following ways:
- Save user preferences
- Preserve session settings and activities
- Provide auto-fill functionality for those who use the Services frequently
- Collect usage data to better understand usage patterns
Data collected using cookies can include:
- Date and time a ‘request’ is transmitted through the Site
- The model of the device making the request

- The web browser used on the device and making the request
- Visiting domain or IP address
- Geographic location

Your data will only be collected through lawful and fair means and not in an unreasonably intrusive way.
How we may use Your Information
PointClickCare uses your EHR data to provide the Services as described on the Site and Terms of Use, as well as to operate, maintain, improve and enhance the performance of the Services and/or create new services.
PointClickCare may use your EHR data to operate and manage the CCC platform, software, and Site; maintain and protect its computer systems; and comply with the law, such as responding to subpoenas and search warrants
Sharing Your Information With Third Parties
As the Services develop, CCC platform functionality may allow PointClickCare to make your EHR data available to third parties participating in the Services that are authorized by you or as necessary to complete transactions you authorize. Additionally, PointClickCare may make your EHR data available to third parties as directed by you.
PointClickCare may disclose your EHR data to PointClickCare Service Providers that provide technical support or other services to PointClickCare related to the Services. All such Service Providers are subject to confidentiality obligations and may only access and utilize your data for purposes of fulfilling their obligations to PointClickCare.
If a third party acquires the assets of PointClickCare related to the Services and its products and services (whether by sale, merger, change of control, bankruptcy or otherwise), your EHR data may be transferred to the new owner(s). In such case, your EHR data would remain subject to the provisions of the PointClickCare privacy policy that was in effect immediately prior to the transfer unless PointClickCare provides you notice otherwise.
Choices you have about how PointClickCare uses your Information
Managing Your Account. You have the following choices regarding the Personal Information you provide to PointClickCare for use:
You may access your CCC account at any time to review your EHR data. To request a change to any of your Personal Information, please contact your Provider or the organization that provided the Personal Information for your CCC account. For technical questions related to the CCC product, please contact your Provider.
Whether or not to grant a Caregiver access to your CCC account is your decision. CCC has no responsibility or liability in connection with any access to, or use of, your account and information by any Caregiver.
By using the Services, you agree that CCC cannot, and has no obligation to, remove Personal Information from your Provider’s, other third-party’s or Caregiver’s records once properly disclosed. You must contact your Provider for any questions on any changes to your CCC EHR.
Terminating Your Account. You may terminate your CCC account at any time by: (i) revoking your consent to sharing your CCC data in the CCC platform and not using the CCC platform; (ii) contacting your Provider and/or following your Provider issued instructions; or (iii) discontinuing your access and use of the CCC.
Data from Children Under the Age of 18
The Services are not intended for use by children under the age of majority. CCC will not knowingly collect information from Providers or site visitors younger than the age of majority.
Security
We employ physical, administrative, and technical security measures to protect against the loss, misuse, unauthorized access and alteration of data and Personal Information under our direct control. When the site is accessed using current browser technology, Transport Layer Security cryptographic protocols protect information using both server authentication and data encryption to help ensure that data is safe, secure, and available only to you.
While these safeguards help prevent unauthorized access, maintain data accuracy in accordance with industry standards and ensure the appropriate use of Personal Information, no security controls are completely foolproof and we therefore cannot guarantee the security of Personal Information.
Your Personal Information is stored on our servers and third-party servers that we use. The safety and security of your Personal Information and EHR data also depends on you.
You are solely responsible for deciding to disclose or transmit EHR data in your CCC account to any external third party and whether to do so via encrypted or unencrypted mechanisms. To further protect the confidentiality of your Personal Information and EHR data, keep your password confidential. Notify your Provider immediately if you believe your password has been breached. Also, remember to log off the CCC site before you leave your computer.
Security Breach Notification Requirements
Pursuant to applicable law, CCC may be required to send you notice of security breaches or suspected security breaches that impact your Personal Information and EHR data.
Definitions:
EHR: “EHR” means Electronic Health Record. The CCC record is an “EHR”.
Personal Information: Personal Information means information about you that reasonably can be linked to you such as your name, health information, demographic information, and/or other identifiers as may be defined under state and federal law. Personal Information may also include but is not limited to your financial information or Social Security Number.
Provider: Provider means a healthcare provider, healthcare practice, or hospital that you authorize to provide information to your CCC EHR.
Services Providers: A Services Provider is an entity that is hired to perform certain functions for PointClickCare to support the development, maintenance, and implementation of CCC. Services Providers may include software or website designers and data storage providers.
“You” or “Yours” means the CCC “User”, being a Resident linked to a Provider, inclusive of any Resident proxy.
Changes to this Privacy Policy
We may update this Policy at any time we deem appropriate to reflect changes in our privacy practices or for other operational, statutory/regulatory or other legal reasons.
While we will endeavor to give reasonable notice of such changes by posting the revised policy on this website, or by other means as appropriate, we do reserve the right, where necessary, to do so without prior notice.
